In order to become a CHIPTECH Inc. customer and have access to our very special pricing and service, we must ask you to take a few moments and fill in these two simple forms.

If you are a "Reseller", we also need a copy of your current annual resale certificate which you must sign in the lower right corner.

PLEASE MAKE SURE:
  1. Indicate on the top left of the application, your Salespersons name, or the name of your contact at Chiptech.

  2. Fill the 3 Reference questionnaire as thoroughly and accurately as possible. It will help us to expedite your application.

  3. If you already have a credit report form, you may send it along with our form. Our form must be at least signed and mailed back to us, even if you are sending your own credit report form.

  4. Fax completed forms to your salesperson at: 954-454-9563 (This step will start your application process)

  5. Mail originals to:

  6. Call or E-mail your Salesperson or contact at Chiptech to let them know that your application is in the mail.
